End of Life Care

In producing an individual end-of-life support plan, we work in partnership with you, your family members, your GPs, and any other professionals supporting you at this time. Together, we ensure that the care delivered is respectful, dignified, and tailored to reflect your choices and needs. Specifically, your plan will focus on the things that are important to you, including:

  • All aspects of your physical, spiritual, and emotional well-being
  • Managing pain and other symptoms to provide maximum relief
  • Emotional support to help cope with complex feelings about death and bereavement
  • Understanding and communicating your wishes in advance to support both you and your family
  • Providing dignity and respect at all times
  • Supporting family members throughout the process
